How often should I see a dentist?

What do I need to bring to my first appointment?

Before your first appointment, our staff may provide forms for you to fill out before your appointment. Typically, included in the dental form will be important information like any allergies you may have, medications you are currently taking, previous medical history, family medical history, and so much more.
 
On top of that, you may need certain documents to have available and bring into the clinic for your first appointment, including:
 
• List of all current medical prescriptions
• Relevant medical history including any illnesses, previous oral surgery, and previous dental care
• Referral letters
• List of questions you may have in regard to the treatment, and our dental technology
• Insurance cards and any claim forms
• Previous dental x-rays within the last 6 months
